Is the apostille fee the same for every document?

In most states, yes — it is a flat per-document fee. A few states waive fees for specific records (for example, some vital records) or add expedite charges. Any such note appears in the state's row and detail card.

Does the fee include shipping or the certified copy?

No. The fee is only for the apostille itself. You still pay separately for the certified copy from vital records or the court, and for your return shipping envelope.
